Treatment Equipment and parameters: PicoSure alexandrite laser (Cynosure, USA, wavelength 755 nm), and PicoWay Nd:YAG dual-wavelength laser (Syneron-Candela, USA, 532 nm band selected). Treatment parameters for 77 tattoos were determined based on factors such as patient age, tattoo color, skin type, and actual treatment endpoint response. Treatment intervals were 2-3 months, with routine follow-up for 3 months after treatment.
How to Choose the Best Treatment Method Based on Tattoo Color?
- Dark Blue or Black Tattoos: Dark blue and black tattoos are generally most sensitive to laser treatment. For patients with light skin tones, Q-switched and picosecond 755nm alexandrite laser, Q-switched 694nm ruby laser, and Q-switched and picosecond 1064nm Nd: YAG laser can all effectively remove them. For patients with darker skin tones, choosing a longer wavelength laser (picosecond 755nm, 1064nm Nd: YAG) is safer.
- Red and Yellow Tattoos: While the optimal wavelength for removing these types of tattoos is picosecond or Q-switched 532nm frequency-doubled Nd: YAG laser, caution should be exercised when using the 532nm wavelength for patients with darker skin tones. This is because shorter wavelengths result in greater melanin absorption by the epidermis, increasing the risk of post-treatment pigmentation changes. However, a few studies suggest that under appropriate treatment parameters, picosecond frequency-doubled 532nm Nd: YAG laser may be safe for red and yellow tattoos. 3. Green Tattoos: The Q-switched 694nm ruby laser is considered the preferred wavelength for treating green tattoos, but those with darker skin should choose carefully. Q-switched and picosecond 755nm alexandrite lasers are also very effective for removing green tattoos.
- Colored Tattoos: When treating tattoos containing multiple colors, multiple lasers are usually needed to achieve the best removal results. Black outlines can be treated with a Q-switched or picosecond 1064nm Nd :YAG laser. If the tattoo ink shows a noticeable bleaching reaction immediately after treatment, the laser wavelength will usually be effective in lightening this color.

Due to their extremely short pulse width, picosecond lasers can achieve extremely high peak power instantaneously, thereby producing a photoacoustic (or photomechanical) effect on the target chromophore, shattering the tattoo dye particles or melanin particles into smaller pieces that are more easily cleared by pigment-loving cells such as macrophages, resulting in a milder inflammatory response. Therefore, picosecond lasers are more effective and have fewer adverse reactions than Q-switched lasers in treating tattoos and most hyperpigmentation disorders.
